FCC REG NO CJEUSA-65828-FA-E FCC ID CJE-O374. I got mine to work with jumpers set to PNP on COM 2, but your COM setting may be different depending on hardware currently in your computer. Windows should recognize your modem as a US Robotics Sportster Voice 33600 Fax RS Rev 2.0
