This driver came from Microsoft. See
www.microsoft.com/isapi/hwtest/nDetail.idc?CN=Aztech+Systems&DT=Modem%2FAnalog&DN=MD6802-U&DB=ISA&SP=PC+97&x=13&y=18
for Win95/Win98 version. Make sure you know your IRQ and I/O for the modem because the defaults did not match mine during the install.
