[FCCID=CNT75MPSBD7] C0-5614WUD, 221519-001CT Spares, 221518-001, Lucent 1640C chipset. Modem was in a Presario 4784. Obtained the driver by using a Restore CD for a 4772 that had the drive image in a Zip file. Extracted the Windows\Inf directory and walked through installing the modem in a different directory copying needed files.
