[FCCID=FB WS-5614PM3C] GVC GPCI-56/CSS1 FCC FB WS-5614PM3C 56k PCI modem with a conexant chipset. There is a sticker over the conexant chip. These are the Windows 98/95 drivers but I have only tested them in Windows 98. When you extract it to a directory and update the driver of the PCI communication device it will only plug and play the Conexant modem enumerator and also make another unknown device. You will have to update the drivers of the new unknown device and specify that you would like to select the device from a list, chose modems and then point towards the directory you extracted the files to. There will be two choices of modems, I chose the voice,data,fax not the speakerphone headset modem and the drivers worked for me.
