This is a generic driver found on Intel site. URL is developer.intel.com/design/modems/support/license/141drv.htm
I've downloaded it to help a friend get their Cirrus logic PNP V34 ISA modem back in service (after reformat and Win95a reload). Intel site indicates it should work with Win 98 and NT4.0 as well. Good luck!
