Boca Research MV34F1 Driver. MV34F1 or maybe MV34FI is inscribed on the card and I assume that's the model.
File Contents: It has a Rockwell R6749-25 chipset. This driver will probably work with all modems using that chip.
Extract the file with something like winzip. It would be easiest to extract it to c:\. Extracting the it will create a Boca folder and dump all the files there. In win 9x go to device manager and click on the modem. Choose to install the drivers. When it asks where to look for the drivers browse to the Boca folder. That's all you need to do for 9x.
For dos (and I assume win 3.1) usage run the install.exe file.
