I got this driver for the Hayes Accura V92 External modem, but it also includes drivers for -

Just unzip the files into a temporary directory and when the modem driver install wizard asks for driver select 'pick driver from list', then 'have disk' and browse to where the files have been unzipped to. Pick one of the files with a .INF file extension then click OK, then pick your modem from the list.
