GVC SF-1156HV/T5 Driver. This is a WORKING driver for this modem. Win98 could not find the Wave driver, but I left it as a yellow '?' and the dialup works fine. Two TI chips on board, TMS320V34PJx2 & TL16CRK600APJM. Have fun!
